The comments on the video said it might have been a botfly. I read a bit about botfly's on Wikipedia and found this equally amazing video of pulling a couple of bot fly's out of a dude. He got them from mosquito bites while hiking in panama. Nice. Those that are squeamish should really appreciate this:
