Every so often when I pull the start rope, it jerks back and just about rips my arm out of the shoulder. What is causing this?

I snapped my winch cable and would like to reattached the hook. Can anybody describe how to do this? What do you use to cut the cable and how is it "tied" together once it is looped around through the hook?
