Sealing bottom plate isn't the answer.... Even sealed if water is getting under at all when it freezes again it will expand and and break the seal in the plate and your back to square 1.

Get a mini excavator dig a sloped trench all the way around... Put in weeping tile.... Blueskin the foundation and plate seam and fill with washed rock...... Problem permanently solved with no lifting or jacking of anything.

A buried drainage system will work great in the summer but at this time of year they will usually be frozen unless it is very large. Culverts freeze solid sometimes.
